I have read and I agree to the Terms and Conditions as laid out on the FACT Adelaide's Website and the Student Handbook, Including  fee structure, refund policy and complaints and appeals policy. I agree that i will check my emails including spam, SMS and if i have not received the emails i will notify FACT Adelaide immediately. 

I agree that i will arrive 30 mins prior to the training start time to provide sufficient time for check in requirements. I understand I am required to provide original ID (Driver License or Passport) that is current and in date. The SA App is permitted however photos of ID are not permitted.

I am not required to self isolate in accordance with state law or travelled overseas or interstate in the past 14 days. I have not showed any signs and symptoms of COVID-19 in the last 14 days. 

I acknowledge that the person that is completing the training is over the age of 18. If the person is under the age of 18 I will complete the under 18 permission form required to take part in the training. Click Here for the under 18 form.
